Case Manager to work 2-3 days per week until end of Sept for LOA. Must have Adult and ER exp. Strong communicaton skills, interacting with patients, physcians, staff, Must have Milliman Guildlines Experience, Clinical assesments, dischanrge planning, care coordination, Windows/Midas computer system.
