JOB DESCRIPTOR:  Licensed Clinical Pharmacist

REPORTS TO:  Team Leader    
DEPARTMENT:Pharmacy

POSITION SUMMARY:    
Provides consultative services to medical, nursing, and pharmacy staff.  Provides prescribed medications, drugs, and other pharmaceutical, and compounds medication, according to professional standards and practices.

POSITION ACCOUNTABILITIES
1. Complies with hospital, department and unit standards, including but not limited to safety, CPR, infection control, quality management, environmental, confidentiality, and patient care.

2. Performs assessment/data collection.

a.Performs on-going assessment of patients and patient care area.

3. Formulates goal directed plan of care based on patient outcomes.

a.Initiates, individualizes, and modifies plan of care.
b.Collaborates with other services.

4. Implements care based on department and hospital standards.

a.Follows established protocols and procedures.

5. Evaluates effectiveness of care.

6. Identifies patient/significant other learning needs and demonstrates measures to meet these.

7. Delegates to appropriate patient care team members.

8. Demonstrates applied knowledge when caring for selected patient populations including care appropriate to the age of patients served on unit.

9. Demonstrates competency in selected skills including, but not limited to:

a.Receives, verifies, and fills all types of medication orders including, TPN,
antimicrobial, chemotherapy, Colony Stimulating Factors and Epogen.
b.Educates staff on medication actions, indications, and side effects.
c.Checks and restocks inventory in accordance with hospital needs.
d.Conducts Drug Utilization Reviews and prepares Drug Review Criteria for review.

10. Demonstrates responsibility and accountability for own professional practice in regards to:

a.Attendance, punctuality, and confidentiality.
b.Positive public relation image and working relationships.
c.Obtaining necessary knowledge and skill needed to maintain competence.
d.Completing all mandatory requirements, including annual safety films, CPR, infection control and annual physical.

11. Participates in the development and attainment of team, department and hospital goals including:

a.Attending and participating in team meetings.
b.Volunteering or accepting assignment for projects.
c.Mission, vision and team ground rules.

12. Demonstrates documentation on department records and forms.

13. Administers and controls narcotics.

14. Performs other related duties as assigned.


POSITION QUALIFICATIONS

Supervision:
Accountable to Team Leader of Pharmacy Team

Minimum Education:    
Graduate of a School of Pharmacy recognized by the California State Board of Pharmacy
An advanced degree such as a Doctor of Pharmacy or Masters of Sciences is preferred

Minimum Experience:    
Must have two (2) years of experience in hospital practice emphasizing clinical pharmacy, pharmacokinetics, and drug information services, or must have completed a clinical pharmacy residency
Good oral and written communication skills must be demonstrated

Skills Required:  
Administrative    
-Ability to deal effectively with many people
-Must be able to prioritize, organize, and coordinate care
-Typing skills

Physical Demands:    
-Prolonged, extensive, or considerable standing/walking
-Lifts, positions, pushes and/or transfers patients, supplies, and equipment
-Considerable reaching, stooping, and bending

Working Conditions:    
-May be exposed to infections and contagious diseases
-May be exposed to odorous or toxic chemicals, gases, or solids
-Occasional pressure due to multiple calls and inquiries
-Requires judgment/action which could result in death of patient
