POSITION QUALIFICATIONS:
Valid CA SLP License.
Valid AHA-BLS.
POSITION PURPOSE:
Evaluates patients, identifies patient problems, plans patient programs, prepares and implements patient education and provides direct patient care to help the individual reach his/her maximum performance. Position is autonomous for clinical provision of care in their area of practice within their scope and follows standards of care.
REPORTING RELATIONSHIPS:
This position reports directly to the Director, Rehab Services.
PRIMARY DUTIES AND RESPONSIBILITIES:
Conducts evaluation of patients referred by a physician.
Analyzes evaluation results to identify the patient's major problems, establish goals with the patient, and develop and adjust treatment programs.
Performs treatments based on evaluation aimed at increasing a patient's functional level. Includes all age specific competencies.
Communicates effectively with physicians, ancillary and nursing personnel, patient and family. Consults with the physician regarding evaluation, treatment, and progress of patients and makes recommendations upon which medical/ professional team can base decisions for care.
Prepares and delivers educational material and training for patients, family members, physicians, staff, students, rehab personnel, other health care personnel, and the general public.
Maintains records of physician referral, the patient's evaluation, treatment plan, treatment given, and patient's response, assessment of patient progress, and other necessary documentation, ie: interdisciplinary tools.
Responsible for supervision of daily operations as needed
Participates in performance evaluations of all rehab personnel
Participates in the planning and developing of existing and new programs, procedures and policies.
Evaluates and makes positive suggestions for change in the environment.

Identifies areas of strength and areas needing improvement through ongoing self evaluations.
Seeks supervisor as appropriate.
Plans and participates in educational programs and workshops to increase professional competence and shares information with staff

INTERPERSONAL RELATIONSHIPS:
Communicates with the patient and family to educate regarding treatment program, techniques, expectations, follow-up and to establish common treatment goals. Communicates with the physician and other members of the patient care team regarding current patient status, progress and treatment programs. Maintains ongoing communication with Rehabilitation Department personnel. Communicates with others in health management regarding Speech Pathologist role in patient treatment and seeks input from them in devising and coordinating patients care.
WORK ENVIRONMENT/HAZARDS:
Lift, move, and assist heavy patients. Lift and push heavy material and equipment. Operate electrical equipment. Exposure to infection, open wounds and other infectious diseases.
